Mr. Beranek:

This is to document our telephone conversation this afternoon.

1.  Facility requires a Class III Operator, but the listed operator only has a 
    Class II license. This was noted in an inspection back in March 2014, and
    the facility responded that they had scheduled the operator for a training
    class in Nov 2014.
2.  Does the wastewater line from the cafeteria have grease trap?
3.  Since facility receives cafeteria wastewater, the renewal permit will
    include and O&G limit.
4.  At least one effluent sample needs to be tested for O&G. Submit a revised
    EPA Form 2E when the data is available.
5.  Need verification of Outfall 001 location (more precise coordinates).
6.  Does the facility still have the “stand-by” chlorination unit, as depicted
    in the line diagram of the system?
